You will find attached the updated “Pre-boarding health declaration questionnaire”, that all passengers/ professional truck drivers/co-drivers travelling to and from Greece must fill in and hand over upon embarkation to the vessel's Chief Purser along with their ticket.

The last question has been added at the updated questionnaire, following relevant instructions from the National Public Health Organization.
